Serpentine belt won't stay put!

Default Serpentine belt won't stay put!

Hello! I'm new here...but weren't we all, at some point?
I have an '89 Astro Van. This morning, I tried installing the serpentine (fan) belt twice, and each time as soon as I turned the engine it knocked the belt off. Everything spins freely (except that one that drives it), and the second time I made sure it was well seated on all the pulleys before turning the key.
So, now, the question: What's the most likely cause for the belt not to stay in place? Second most likely cause? As many possibilities as I have to go through to figure it out/make it work?

Welcome to the fourm,
Ensure the belt tensioner is fully seating and keeping the belt tight
verify that the belt is the correct size
rather then start the vehicle after install, bump the starter to move the belt a few inches at a time to ensure its seated
Check all pulleys for firm bearings and no play

Look to see if the tensioner is leaning out.
